Make a referral
Referrals can be made by Care Coordinators, Social Workers, Occupational Therapists, Resettlement Managers, Commissioners or Social Services for those people who do not receive support through a Care Plan Approach (CPA).
Eligibility criteria
- Aged 18+
- Any gender identities
- A CPA report and recent risk assessment
- Service User is prepared to engage with support services provided
- Service User’s current ADL (Assessment of Independent Living Skills) identifies a degree of independence
- Commitment to address support needs e.g. Mental health, learning disability, autism, drugs/alcohol, gambling, anger management, education and leisure activities, etc.

Our goal is to support residents to progress and live an independent life within 2 years. However, we understand that some individuals may have complex needs and require more time to complete this transition, The length of stay is dependent on the progress of the individual.
